Administrative Case Manager - Part Time
Position Summary
Working under the ACM Director, this position provides administrative case management to children and young adults in Montgomery County. Primary responsibility is to assist clients in obtaining resources and benefits and connections to other County programs. The ideal candidate would be reliable, independent, organized, compassionate, and have strong communication skills.
Work schedule: 24 hours per week, flexible schedule, during general business hours (8am-5pm). Some evening hours are required.
Required Qualifications
- Bachelor’s Degree in Social Work, Psychology, or a related field
- Knowledge of community systems and resources
- Prior experience working with children and families
- Respond to all incoming ACM calls and emails, and address all new referrals
- Document client contacts in electronic health record
- Register all clients in Montgomery County portal
- Support families in administrative case management needs; such as assisting in completing Medical Assistance application, helping connect families to other resources or providers, coordinating cradles to crayons, diaper, and other donations and distributing to families
- Working with families over the phone, email, or in the home or community
- Ensure client charts via EMR comply with program regulations and agency standards
- Attending Abington Hub and other community meetings
- Attend community events to share info on ACM and other CFF services
- Maintain weekly billable expectation
- Use and implement the 5 core values of Trauma-Informed Care in all work practices – Safety, Trustworthiness, Choice, Collaboration, & Empowerment
- Other administrative duties as assigned

- Work performed in a standard office environment as well as out-of-office settings to include client homes, schools, court, medical offices, and other community settings, as needed.
