Position Summary:

The RTF Referral Management (RRM) program is a site-based program contracted by Delaware County Office of Mental Health to provide an administrative process of coordinating RTF and CRR-HH treatment for youth who have been approved by Delaware County Behavioral HealthChoices for this level of care. The RRM Specialist is an experienced professional who specializes in collaboration and coordination with child-serving systems in Delaware County. The RRM Specialist will ensure identified youth within the HeathChoices system managed by Delaware County and CCBH are admitted to appropriate Residential Treatment Facilities (RTF) and CRR-HH when referred and approved for treatment at this level by coordinating provider searches. The RRM Specialist position is Full-time, 40 hours/week, M-F 8:30-4:30 pm, including required one day per week in the office.

Required Qualifications:

  • Bachelors degree in Human Services, Psychology or a related field
  • Two or more years of experience working in child serving systems
  • Two or more years of direct services experience in the human services system

Preferred Qualifications:

  • Master’s degree in Human Services, Psychology or a related field.
  • Demonstrate strong communication skills.
  • Display excellence in multi-system management and collaboration.
  • Have a basic understanding of RTF and CFF-HH referral process.
  • Possess group facilitation, time management, and organization skills.
  • Display proficiency with computer applications including Microsoft Word, Excel, DocuSign, and PowerPoint.
  • Possess prior crisis management experience.
  • Have prior knowledge of TIC practices.

Program Specific Responsibilities:

  • Manage all referrals for RTF and CRR-HH for youth residing in Delaware County once they have been approved by CCBH.
  • Maintain current and accurate list of RTF and CRR-HH providers.
  • Work closely with Delaware County Office of Mental Health (OMH) and Community Care Behavioral Health (CCBH).
  • Participate in ISPT (Interagency Service Planning Team) meetings for identified youth who are being recommended for RTF and CRR-HH.
  • Facilitate the provider search for all CCBH-approved RTF and CRR-HH referrals until an appropriate treatment facility has been identified and the youth has been successfully admitted. 
  • Coordinate Consents and Releases via DocuSign for each referred youth.
  • Ensure connection to prescribed services through collaboration with the referring provider, caregiver, OMH, CCBH, and other Delaware County-involved systems; proactively addressing barriers to successful treatment.
  • Coordinate with RTF and CRR-HH staff seeking approval/denial of admission.
  • Track the disposition of each referral and regularly report the status of referrals to all appropriate stakeholders.
  • Submit weekly tracking reports to OMH and CCBH.
  • Coordinate/Host RTF Referral Case Reviews with treatment team members to problem-solve barriers to admissions.
  • Participate in monthly case reviews with OMH and CCBH.
  • Attend Quarterly Implementation Meetings with key stakeholders and use feedback from meetings to improve service provision and administrative processes.
  • Follow all guidance regarding the RTF referral management process from OMH and CCBH.
  • Ensure youth are referred to appropriate RTF settings based on the needs of the youth, their voice, and choice within available resources of the CCBH network.
  • Provide direct linkages to services and resources to families.
  • Monitor the youth through 60 days ensuring a successful transition to prescribed treatment facility has occurred.
  • Create and manage electronic client charts.
  • Document all communication, case notes, emails, and conversations relevant to the referral process in client file and EHR system.
  • Support RTF Referral Management Director in fostering a supportive team environment and approach to services.
  • Participate in supervision with the RTF Referral Management Director.
  • Use and implement the 5 core values of Trauma-Informed Care in all work practices - Safety, Trustworthiness, Choice, Collaboration, & Empowerment.
  • Other duties as assigned.
Work Environment:
  • Manage a hybrid work schedule Monday-Friday from 8:30-4:30pm, with one day required in the office setting.
  • May be required to work in client homes, schools, or other community-based settings.
