Mental Health Case Manager

Child and Family Focus is hiring a Mental Health Case Manager in Chester County! The First Episode Psychosis (FEP) program uses a Coordinated Specialty Care approach to support young people experiencing their first episode of psychosis. Services include medication management, therapy, family education, case management, supported education and employment, and peer support. The program focuses on empowering individuals to guide their own recovery, build relationships, and successfully reintegrate into school, work, and the community!
The Case Manager develops and coordinates services based on individual service plans, providing case management, service coordination, supportive counseling, and advocacy to help clients achieve recovery goals and build self-advocacy.
This role works within a multidisciplinary team to deliver community-based support to a caseload of up to 30 adolescents and young adults (ages 15–30) experiencing their first episode of psychosis.
Program Specific Responsibilities:
- Assume case load of up to 30 clients
- Assist in the completion of comprehensive assessments and treatment plans for each client
- Complete a written strength assessment, develop and implement a written service plan for each consumer assigned to the caseload
- Provide services that diffuse or resolve crises in order to prevent unnecessary use of emergency rooms, in-patient services and mental health court
- Make home, school, hospital and community based visits at a minimum of every 30 days (or more often as determined by the consumer’s level of care)
- Access community resources and child-serving systems in Chester County
- Achieve billable hour requirements per week
- Transport clients and families to various appointments based on caseload as needed
- Document supervision and case activity through ETO, progress notes and other program specific paperwork by typing or handwriting
- Ensure that program files are organized and paperwork is filed into client specific charts
- Provide on call rotation for 24/7 mental health crisis support as needed
- Use and implement the 5 core values of Trauma-Informed Care in all work practices - Safety, Trustworthiness, Choice, Collaboration, & Empowerment
- May perform other related duties, when required or assigned
Required Qualifications:
- Bachelor’s degree in Human Services, Psychology, Social Work or a related field
- One or more years of direct clinical experience in mental health services working with children, youth and families
- Maintain valid driver’s license and the ability to travel in your own vehicle to off site locations as required by caseload
Preferred Qualifications:
- Knowledge of community resources in Chester County
- Bi-lingual in Spanish Language
- Crisis management experience
- Experience working with the Schizophrenia Spectrum
Compensation, Benefits and Schedule:
- Hourly rate of $19-19.50 an hour
- Varied work schedule, evening and weekends may be required based on client needs
